Belmont Terrace Nursing and Rehabilitation Center was the largest nursing home in Boone County for the first quarter of 2026, according to the Centers for Medicare and Medicaid Services (CMS).

The facility offered 151 beds, three above the county average in that period, and provided care for an average of 120.9 residents.

The CMS assigned Belmont Terrace Nursing and Rehabilitation Center an overall rating of 1 out of 5, trailing Kentucky’s nursing home average rating of 3.

Florence Park Care Center ranked second in Boone County with 150 beds, while Boonespring Transitional Care Center, LLC was third with 143 beds during the same quarter.

By the first quarter of 2026, Belmont Terrace Nursing and Rehabilitation Center had incurred $35,980 in six fines and had seven penalties on record.

Altogether, Boone County, Kentucky, was home to three for-profit nursing homes.

Estimates from the U.S. Census Bureau indicate the older adult population in the U.S. continues to climb as younger groups decline. From 2020 to 2024, the number of Americans age 65 and older grew by 13%, while those under 18 decreased by 1.7%. The nation’s older adult population has expanded significantly over the past century, rising from 4.9 million in 1920 to over 61 million in 2024.

A study published in 2021 estimates that nearly half of people in this age group, about 28 million, could need long-term services and support, with more than a third projected to require nursing home care at some point.

“Living a long life is something that many of us want and could get,” said Jesse Slome, executive director of the American Association for Long-Term Care Insurance. “But when we live a long life, the chances of us needing long-term care increase exponentially. But when you need that type of care, there are limited options.”
